Elko Southern Bands is an ambulatory clinic providing many people with essential health care. Staffed by one physician, two nurse practitioner, one social worker and one dentist, this AAAHC Medical Home and AAAHC Dental Home accredited family practice clinic offers direct medical and dental services. The total number of staff consists of 45 employees. Visiting specialties include audiology and rheumatology. We also offer tele-health services including behavioral health services, nutrition and rheumatology. Purchase/Referred Care services are also coordinated by referring to hospitals and clinics both locally and in nearby cities. On-site pharmacy services are available including a robotic prescription dispensing system; automated telephone refill line and mail out pharmacy services. A 24/7 Nurse Advice Line is another resource available to our community. Southern Bands Health Center uses an Electronic Health Records system with integrated Behavioral Health Services in addition to an Electronic Dental Record system.

Job Description: NP/PA: Primary Care/Family Medicine
Assignments include providing comprehensive and continuing health and medical care. Provider(s) conducts urgent/emergent services, making observations appropriate to the patient’s underlying disease and treatment plan. Provider(s) review the patient’s medications, diet therapy course, and management plan. The provider modifies regimen as indicated. Provider instructs and counsels patients and family members as required. Contract provider(s) refer patients to appropriate contracted medical (or other) facilities, providing full clinical information for care and diagnostic procedures that cannot be adequately provided at local IHS facilities. Coordinates and integrates information obtained into the ongoing Health Care Program at the Service Unit and assures timely follow-up care as required. Contract physician(s) also complete all necessary paperwork for Contract Health Services for all patients referred. Contract Provider(s) work closely with allied health professionals (i.e., nursing, Community Health Nurse, Social Workers, Health Educator, etc.) to utilize and coordinate the services of Health Care Professionals in the management of the patient’s medical, psychological, and social problems.
