Setting: 60-bed inpatient hospital + ambulatory care center (rural, ~35,000 patient population)

Requirements: 

• Board certified in Internal Medicine, Family Practice, or Med/Peds (ABIM, ABFM, or ABP). New grads may be board-eligible only.
• Completed US residency — 36 months FP or IM, or 48 months IM/Peds.
• Minimum 6 months of Hospitalist experience.
• No lapse in clinical practice greater than 6 months leading up to start date.
• No prior loss or suspension of licensure in any medical profession — ever.
• Active, unrestricted state medical license (any US state, DC, or Puerto Rico).
• Active, unrestricted DEA with Schedules II–V.
• Current BLS + ACLS. Med/Peds candidates also need PALS + NRP.
• Clean NPDB report — must be pullable for submission.
• Willing and able to commit to 7 on / 7 off for at least 4 consecutive weeks.
• All malpractice suits and board actions disclosed up front (we will list them in the submission).

Role Overview — What the Provider Will Actually Do

The provider delivers full inpatient and outpatient hospitalist coverage at CCHCF, including:

• ER and Urgent Care coverage
• Rounding on ACU and SCU (and PCU/Nursery for Med/Peds)
• Care of critically ill patients; coordinate with IHS and outside specialists
• H&Ps, daily progress notes, discharge summaries, admission orders
• Order and interpret diagnostic studies; prescribe and adjust treatment
• Make appropriate referrals to contracted facilities for care not available locally
• Document 100% of encounters in CCHCF's EHR within 4 days of the encounter
• Participate in department and medical staff meetings, peer review, OPPE/FPPE
• Comply with Joint Commission, CMS, HIPAA, and Privacy Act standards
• Complete CCHCF-provided cultural awareness and child-protection trainings within 30 days of onboarding

• Nearest larger towns: Gallup, NM (~90 mi) and Farmington, NM (~115 mi) for Walmart, Safeway, Albertsons, etc.
• Patient population: ~35,000 Navajo patients; many travel 100 miles round-trip for care; many elderly patients speak only Navajo. Cultural sensitivity is a real expectation, not a checkbox.
